WINTER SPORTS ATHLETIC PACKETS DUE BY OCTOBER 16, 2009
The MHS Athletic Packet is available above under Links / Forms.
Down load the ***09-10 Athletic Packet. Completed packets for Winter Sports are due by Friday October 16, 2009. Do not be late!! Yes everything must be filled out completely. Athletes with incomplete forms will not be allowed to participate until the forms are complete and approved by the Athletic Director.
If you turned in all of your paper work for a fall sport you only need to turn in the MHS Athletic Eligibility form for a 2nd or 3rd Sport and a Parent Waiver to not attend additional meeting. You can only use the waiver if a parent personally attended the fall meeting. Parent's who sent representatives may not use the waiver form.
Please remember Winter Sports are basketball, soccer, and wrestling.
